Registration with multiple agencies is not permitted. Clients must complete this application and provide it along with copies of eligibility documents (ID and proof of disability or income documents for low-income qualification) to participate.Mail this completed application and eligibility documents to: RIDES Call 713.313.RIDE (7433) to schedule an appointment to apply in person at our offices. Once you are approved and registered, you are eligible to receive discount travel tickets that may be used with any of the designated transportation companies. No personal checks accepted — Money order or cashier’s check accepted for purchases by mail. Cash, money order or cashier’s check accepted for in-person purchase. Each book of ten tickets is $25.00. |
